Project Overview
HatchWorks partnered with Spring Hills to strategize, design, & develop a population health software platform that would scale with the organization & meet long-term business needs. Work began by defining the business case, market opportunity, & understanding the nursing staff & patient management process. HatchWorks created a proof of concept, product roadmap, wireframes, & an interactive prototype.
Implementation of the population health program began by defining the assessments & workflows necessary to positively impact patient outcomes. These workflows resulted in tasks & assessments for the medical team managed in spreadsheets & static documents. As their medical team grew, they realized a more automated system was needed to manage patient information & clinical tasks.
HatchWorks partnered with Spring Hills to design & build their population health solution to support configurable workflows based on user roles & medical scenarios & to support different types of health system customers & patient care periods. HatchWorks interviewed stakeholders to understand how the solution would support their business & define the targeted users & the critical tasks they would complete. This led to comprehensive user workflows & prototypes, the basis for the logic behind the task generating functionality & the UX of the application.
The workflows created helped Spring Hills internal processes become more efficient. The design system established a comprehensive user experience & clean, easy-to-use interface that set the stage for the continued development of the MVP application. The solution immediately allowed Spring Hills to transition from management of patients via spreadsheet to a custom application that manages tasks & patient assessments. The application has reduced the 90 day readmission rate for the population that Spring Hills manages to under 10%, where the state average (NJ) is over 20%.Client
